If you have elected 401(k) provisions, be sure to either fax or mail back to us the salary deferral elections for all employees electing to contribute to the plan.
You may pay your deposit either by check (mail to ALI Actuarial & Retirement Plan Services, 5106 Paperbark Court, Durham, NC 27713) or by credit or debit card by clicking on Payments.
